The Crystal Ridge Care Center Citation

The former Meadow Vista Manor nursing home now called the Crystal Ridge Care Center in Grass Valley been fined $75 thousand dollars following a death at the center on Dorsey Drive.

Crystal Ridge, was also cited following a CDPH investigation that found the facility failed to use a mechanical lift as directed in its own policy resulting in the fall and the death of the unnamed resident February 10. The resident died 4 days later from injuries that included a large gash on the head and 3 broken bones in the neck after falling into an empty bath tub.

Ralph Montonyo spokesman with the department of public health says a fine and plan of compliance are expected from Crystal Ridge.

“They will have to file what is called a ‘plan of correction’ and will submit that to us and we will review their plan and also submit it to our counterparts and both agencies must approve that plan of correction and then we will conduct a surprise inspection to ensure that everything within the plan of correction has indeed been corrected.”

The citation Crystal Ridge Care center received is a class “AA” citation, the most severe penalty under state law.
