It’s all done online at 2-1-1 connecting-point dot-org. If you don’t have access to the internet, or just want help with the form, you can just dial 2-1-1. People who are experiencing symptoms that could be related to Covid-19, or have a loved one who does, are being asked to respond.

The data forms graphical information that is constantly updated on the website. The website is 211connectingpoint.org/covid-19/project.
