Your voice is needed!
Nevada County Public Health Department is leading a collaborative process to develop a comprehensive health assessment of the community, its health challenges and its resources. The intent is to have a deeper understanding of what issues residents feel are important, how quality of life is perceived in
our communities, and what community assets we have that can be used to improve community health.

We are asking community members to complete an online survey. The questions are about quality of life, health care, children, seniors, economic opportunity, safety, social support, community health, stress, and risk behaviors in the community. Some basic demographic information is collected to allow us to learn what matters to the different individuals and communities completing the survey. It is important to us that youth and adults of all ages residing in Nevada County complete the survey – we want to hear from everyone!

The survey is available in English at:
https://www.surveymonkey.com/s/NCPHDCommunityHealthSurvey or in Spanish at
https://www.surveymonkey.com/s/NCPHDSurvey_Spanish.

At the end of the survey, we offer an option to be entered into a drawing for your choice of one of three
prizes:
$25 gift certificate to Safeway
$25 Amazon.com gift card
$25 Broad Street Books

No access to a computer or mobile device? Need paper based surveys for your class or group? Just let us know and we’ll work with you. Your voice is needed! If you have any questions or are interested in
additional opportunities for participating in the community health assessment, please email or phone:
